You will need the following items in order to get your child enrolled for our program:
*Copy of your child's Birth Certificate
*Copy of your child's Social Security Card
*Copy of your child's current Immunization Record
*Registration Paperwork (packet from the school)
*Medical Form that must be signed by a physician or certified nurse practitioner. This medical form must be updated every 13 months.
*2 Forms of Proof of Residency (please call for more information)

Enrollment for the next school year opens in February.
